Thanks to pronex, we have proof that the plate I saw on the DVD was in fact on the bench The Oracle and The Architect sat at the end of Revolutions.

If you would like to see where I got this from, simply goto the matrix website, on the front page where you click on high bandwidth, low bandwidth, or lite version, scroll down and click on "zion archive". When that page loads, click on the menu on the left and then scroll down to "park". You can view images of the park that the Oracle sat in at the end of M3. Hope this helps!

The way I interpret this is that while Neo's human part (Thomas Anderson)is dead, the program part of him (Neo, or "The One", if you wish) is still alive.

Perhaps "Thomas Anderson" died (or disappeared?) within the Matrix when he was unplugged. So his friends within the Matrix placed the plaque.

The marker on the bench said "In Memory Of Thomas Anderson". It isn't necessarily a remembrance of his passing life. it could simply be a tribute to his accomplishment. Why does everyone insist on ushering Mr. Anderson off to the grave. The freakin' lady said they might see him again someday.

Technically, Thomas Anderson has been dead since he was unplugged.
Neo is the one who was alive.


But, there have been a couple of threads about this started before - good observation.

Revolution is the birth of equality and the antithesis to oppression...

What so important he did as Anderson [not Neo] that qualified him to have a plaque on a bench ... prolly it's a movie reference to the guy's passing away which I think is lame never the less.
